Questionnaires on Embodia

Welcome to Questionnaires (HEP) on Embodia!

This course will guide you through the Questionnaire features available on Embodia. 

Questionnaires is an umbrella term used to describe a broad range of questionnaires including but not limited to outcome measures, intake forms, consent forms, screening forms, and satisfaction surveys. You can choose to use the templates available on Embodia including a wide range of validated outcome measures, or you can create your own.

Embodia scores and keep track of questionnaire data for you. Results are displayed on easy-to-view graphs.

Patients fill out, review, provide consent and/or sign questionnaires shared with them from their secure Embodia portal, saving practitioners and patients effort, time, and money. 

Questionnaires on Embodia require a Tier 2 or Tier 3 membership. You can learn about our membership options here.

If you already have a Tier 2 or Tier 3 membership, you can perform all of the actions we outline in this course. We suggest watching/reading each subsequent lesson and then performing the task on your account. This will help solidify what you're learning.

*Note: A Tier 3 membership requires a clinic account. 

Embodia offers individual memberships and clinic memberships. You can sign up as an individual practitioner by following the steps in the help article, Signing up as an individual practitioner, or as a clinic by visiting the help article, Signing up as a clinic.
